What This Book Argues

Competent organizations keep failing at AI in the same recognizable ways. The technology usually works. The people are usually capable. The oversight is usually present on paper. The failure happens in the structure that connects them.

Outpaced by AI names thirteen failure modes that account for these collapses. Each is examined through documented cases, then traced to the structural conditions that produced it and to the oversight assumptions that proved too slow to catch it.
